Map locates Italian cruise ship adrift with no power in Indian Ocean

NAIROBI, Kenya (AP) ? A crippled Italian cruise ship being towed in the Indian Ocean with more than 1,000 people aboard and no air conditioning now won't reach land in the Seychelles until Thursday, officials said.

The cruise ship company said that food, satellite phones and VHF radios would be brought to the Costa Allegra by helicopter. Photos released on Tuesday showed hundreds of people milling on the ship's outside decks, and officials said passengers would sleep there as well instead of in their unlit cabins.

The Costa Allegra has 636 passengers and 413 crew members on board: Among them are 212 Italian, 31 British and eight U.S. passengers. Four of the passengers are children ages 3 or younger.

The ship lost power Monday after a fire in its generator room, which knocked out power to the ship's engines as well as to its lights and air conditioning.

Cruise ship officials had said that they would be taking the stranded travelers by Wednesday to Desroches, a small, exclusive coral-lined island in the Seychelles. However, they later said Tuesday that they would instead bring them to the main Seychelles island of Mahe.

The cruise ship company said the change was done for safety and logistical reasons, and that the Allegra would reach Mahe early Thursday. Two tug boats have joined a French fishing vessel to tow the cruise ship.

The fire aboard the Costa Allegra comes only six weeks after one of its sister ships, the Costa Concordia, hit a reef and capsized off Italy, killing 25 people and leaving seven missing and presumed dead.

No one was injured in the fire Monday, but the blaze set the cruiseliner adrift at sea in a region where Somali pirates prey on ships.

Both ships are operated by Costa Crociere SpA, which is owned by the Florida-based Carnival Corp.

However, company officials rushed to play down concerns.

The Costa Allegra is adrift "and being pushed by the current. It is stable and upright," Giorgio Moretti, the director of nautical operations for Costa Crociere SpA, told reporters in a conference call late Monday from company headquarters in Genoa, Italy.

"It's a big ship and to tow it, to move it across the waters, is a heavy task," said Seychelles presidential spokeswoman Srdjana Janosevic. She said that everything is calm on board the cruise ship and that no one is hurt.

Italian Coast Guard officials said emergency generators were keeping the ship's control room illuminated and communications equipment such as radios running.

The Allegra, whose Italian name means "merry," or "happy," had left northern Madagascar, off Africa's southeast coast, on Saturday and was cruising toward Port Victoria when the fire erupted. Costa said the Allegra had been due in Port Victoria on Tuesday.

The general region where the cruise ship was adrift ? off the coast of Tanzania ? has seen a rash of attacks by Somali pirates. In 2009, an Italian cruise ship with 1,500 people aboard fended off a pirate attack in the Indian Ocean far off the coast of Somalia.

Moretti said an armed nine-member Italian military team on anti-pirate duty was aboard the Allegra, but he insisted the maritime region where the ship was now "isn't a high risk area for pirates."

"If pirates attack, the armed guards on board will respond. But as far as I am aware, no pirates have been sighted in the area," said Janosevic.

Moretti said 15 Costa engineers, technicians and other officials were flying to Mahe in hope of reaching the Allegra by air to repair its generators.

Mortgage and home equity lines of credit (HELOC) balances fell a combined $146 billion, a sign that consumers continue to reduce housing related debt.

After a mild uptick in the third quarter, total household delinquency rates resumed their downward trend in the fourth quarter. The report finds that $1.12 trillion of consumer debt (or 9.8 percent of outstanding debt) is currently delinquent, with $824 billion seriously delinquent (at least 90 days late). Meanwhile about 2.2 percent of mortgage balances transitioned into delinquency during the fourth quarter, resuming the recent trend of reductions in this measure. However, delinquency rates remain elevated compared to historical figures.

"While we continue to see improvements in the delinquent balances and delinquency transition rates this quarter, there has been a noticeable decrease in the rate of improvement compared to 2009-2010," said Andrew Haughwout, vice president and economist at the New York Fed. "Overall it appears that delinquency rates are stabilizing at levels that remain significantly higher than pre-crisis levels."

As of December 31, 2011, total consumer indebtedness was $11.53 trillion, a reduction of $126 billion (1.1%) from its September 30, 2011 level. Mortgage balances shown on consumer credit reports fell again ($134 billion or 1.6%) during the quarter; home equity lines of credit (HELOC) balances fell by $12 billion (1.9%). Household mortgage and HELOC indebtedness are now 11.0% and 11.7%, respectively, below their peaks. Consumer indebtedness excluding mortgage and HELOC balances again rose slightly ($20 billion or about 0.8%) in the quarter. Consumers? non-real estate indebtedness now stands at $2.635 trillion. Student loan indebtedness rose slightly, to $867 billion.

The second graph shows the percent of debt in delinquency. In general, the percent of delinquent debt is declining, but what really stands out is the percent of debt 90+ days delinquent (Yellow, orange and red). The percent of seriously delinquent loans will probably decline quicker now that the mortgage servicer settlement has been reached.

In December 2010, President Obama and Congress reached an agreement whereby the Social Security payroll tax would be cut by two percentage points in 2011?from 12.4 percent to 10.4 percent?as a temporary stimulus measure. The president later proposed that the cut be extended and expanded in 2012. Before Congress went home at the end of 2011, it passed a 60-day extension of the two-point tax cut. The payroll tax cut has now been further extended to last throughout 2012 at least.

Most public discussion of the payroll tax cut has pertained to its efficacy (or lack thereof) as economic stimulus. Its greater policy significance, however, lies in another provision tucked into the same law. This provision is now transferring more than $215 billion in general tax revenues (e.g., income taxes) into the Social Security Trust Fund to make up for the reduction in payroll tax revenue.

In other words, the payroll tax cut is not really reducing the amount of tax revenues committed to Social Security. All that it actually does is to shift the Social Security financing burden from covered workers to others?most notably, to those Americans who pay income taxes. This is a transformative change to Social Security, reflecting goals for broader income redistribution now ascendant on the left end of the American political spectrum.

The new policy ends the longstanding requirement that Social Security expenditures be limited to the total amount of its tax collections (plus interest). And, by subsidizing payments with income taxes, it ends the idea that the Social Security benefits are fully ?earned? by recipients.

The recent shift to income-tax-financing embodied in President Obama?s payroll tax cut policy cannot be said to represent a bipartisan agreement with this new policy view. Instead, the payroll tax cut was first proposed on the basis that it was necessary for economic stimulus, and later extended with the argument that doing otherwise would impose a painful tax increase on working Americans. The fact that the law also contained a provision to begin significant subsidization of Social Security with income taxes only belatedly gained the notice of the press, and not yet of the general public.

A critical milestone has nevertheless been passed. Beginning in December 2010, and continuing through to the present time, the federal government has embraced the policy of committing income taxes to subsidize benefits beyond those that Social Security itself can finance. Unless this policy is rapidly reversed, readers of this article who pay income taxes should brace themselves for the substantial new taxes they will soon be paying to bail out Social Security.

ScienceDaily (Feb. 19, 2012) ? Using a mathematical formula that carefully measures the degree to which HIV infection of immune system cells is stalled by antiretroviral therapy, AIDS experts at Johns Hopkins have calculated precisely how well dozens of such anti-HIV drugs work, alone or in any of 857 likely combinations, in suppressing the virus. Results of the team's latest research reveal how some combinations work better than others at impeding viral replication, and keeping the disease in check.

"Our study results should help researchers and clinicians develop simpler treatments, using either existing or new drugs, for people who are just starting therapy or people who have already tried and developed resistance to another combination," says senior study investigator and infectious disease specialist Robert Siliciano, M.D., Ph.D.

Siliciano, a professor at the Johns Hopkins University School of Medicine and a Howard Hughes Medical Institute investigator, and colleagues constructed the measurement tool, called the instantaneous inhibitory potential, or IIP, in the laboratory several years ago by analyzing the shape of drug dose-response curves in human immune system cells infected with HIV. They found that the curves' steepness reflects the extent to which small increases in the amount of drugs can further suppress attempts by the virus to bounce back, reproduce and spread.

Researchers say their latest study findings, to be published in the journal Nature Medicine online Feb. 19, along with other recent studies, provide valuable information to physicians about the potential strength of different combination drug therapies, and can help in streamlining and tailoring so-called highly active antiretroviral therapy, or HAART, to as few possible drugs as needed. Several hundred thousand of the more than 1 million Americans living with HIV disease are currently using HAART to fight the disease.

Among the latest study's key findings was that the most potent drug combos included the drugs efavirenz (a non-nucleoside reverse transcriptase inhibitor) and darunavir (a protease inhibitor.) According to the Hopkins team's calculations, the drug mix suppressed viral replication by more than a trillion times, enough to prevent infection of every single lymphocyte, or immune system cell, of which there are a trillion in the body.

The least-powerful drug was found to be one of the oldest anti-HIV medications, d4T, or stavudine (a nucleoside analogue reverse transcriptase inhibitor), which had the power to suppress viral replication by less than 10 times if used on its own (although, Siliciano points out, it works much better when taken in combination with other drugs.)

Siliciano says the most widely used combination, a single pill known as Atripla, consisting of tenofovir disoproxil fumarate (a nucleotide analogue reverse transcriptase inhibitor), emtricitabine (a nucleoside analogue reverse transcriptase inhibitor), and efavirenz, was able to reduce viral replication to as few as one in a billion.

Siliciano points out, however, that any drug combination which suppresses viral replication to the degree that out of every 100,000 lymphocytes exposed to the drugs, only one lymphocyte is likely to be infected (for five tenfold reductions) -- is sufficient to keep the disease in check, so long as people take their medication as prescribed.

"This means that overall access to anti-HIV medications could also improve as we develop simpler combinations of fewer drugs to achieve near total suppression," says Siliciano. Less than 7 million of the 34 million people worldwide infected with HIV are taking antiretroviral therapy, he notes.

The Johns Hopkins team based its new calculations on five years of analyzing just how antiretroviral drugs hinder key steps in HIV's life cycle, preventing it from replicating and infecting other immune system cells.

Scientists have for decades focused on multiple drugs targeting different enzymes that are key to the viral life cycle, thinking that multiple barriers along the chain could best halt replication.

Although the strategy worked, scientists had, until now, no theory to explain why some drug combinations worked well and others did not. Indeed, they point out, one of the newest classes of anti-HIV medications, so-called integrase inhibitors, did not work well as single drug treatments in laboratory experiments, but were highly effective in people when combined with other drugs.

Siliciano says that as a result of the Hopkins team's latest research and another of their recent findings, published in Science Translational Medicine in July, experts can finally demonstrate how different drug combinations disrupt and halt viral replication.

Researchers found that the steepest curves occurred when the drug targeted a stage in HIV's life cycle, in which many copies of viral enzymes, were needed. Citing protease inhibitors as an example, Siliciano says several copies of protease enzyme are needed to cleave the virus into hundreds of working parts before HIV can infect a new immune system cell. He goes on to say that "a level of inter-enzyme cooperation" is happening, specific to each stage of HIV replication.

"Our research shows that drugs like protease inhibitors really work like an on-off switch," says Siliciano. "Above a certain concentration, these drugs completely turn off viral replication. When you have only one copy of a viral enzyme needed in any key part of HIV's life cycle, a little more drug won't give you a lot more suppression; but, when you have more than one copy of enzyme needed for viral replication, then the dose-response curve for the drug will be a lot steeper, and a little more drug will completely shut off viral replication, which is what we want.

"It's gratifying to finally have a consistent metric for evaluating HAART medications that offers reliable information on how well they work in stopping HIV replication, and which also gives us a baseline target for suppression at less than one in 100,000 immune cells becoming infected in the presence of any drug combination," he adds.

The Johns Hopkins inhibition index was first developed to compare the level of viral inhibition from different drugs in different classes and to show how they could be graded.

Having measured the different potencies of many drugs, Siliciano conducted his next set of lab experiments to focus on the explanation behind different strengths of viral inhibition. The scientists measured the changes in the dose-response curves, plotting the results on graphs and comparing the sloping curves for each drug or combination of drugs.

Funding for this study, conducted solely at Johns Hopkins, was provided by the Howard Hughes Medical Institute and the National Institute of Allergy and Infectious Diseases, a member of the National Institutes of Health (NIH).
